When you're starting your coaching business, you don't want to have to re-invent the wheel every time you need to produce a form or template. To make things a little easier, we've collected some sample documents which we share with our clients.

All documents are in MS-Word format to allow you to customize them for your use. Members can click on a document to view it, or right-click the document and choose Save Target As... to save the document to your computer's hard drive.

Intake Forms and Materials

Sample Welcome Packet 1 - includes personal information form, a sample coaching agreement and a place for the client to indicate areas of focus
Sample Welcome Packet 2 / Intake Questionnaire 1 - this welcome packet includes both areas to collect client information, and an intake questionnaire as well
Intake Questionnaire 2 - Some additional questions you might ask in a client questionnaire (in a client's welcome packet)
Sample Coaching Agreement 2 - also includes an information disclosure request (offering the client the ability to "opt out" of this clause)
Personal Information Disclosure Request - this form requests the that you, the coach, be granted the right to disclose certain information about the client for the purposes of supporting your application(s) for certifications or accreditations in the future. This information can sometimes be difficult to obtain long after the fact
Credit Card Authorization Form - sample agreement to allow you to make recurring charges to a client's credit card
Design and Discovery Client Worksheet - this form can be given to clients for use during their intake session. It will help them better understand the structure of the session, as well as give them a place to take notes about particular terminology that will be important throughout the coaching process
